These forms are not intended for symptoms, diagnoses, treatment requests, appointment details, insurance information, or any other sensitive medical information. Submitting a form does not create a physician-patient relationship.

How information is used

• To provide launch, network, and membership updates you requested.
• To respond to physician-network inquiries.
• To understand general interest in the developing Inner Circle service.

Inquiry storage and follow-up

Inquiry details are saved in a private website database and an access-restricted Google Sheets tracker. The Inner Circle team uses these records to review inquiries, organize follow-up, and respond by email through its business Gmail account.
